For Wig theorists, here's a clue from the tapes team during Fred's Drops special

During the Fred special on drops they used the OJ interview. At the very start Howard Apologizes for wearing a Babushka on his head, states he's going though chemo. This would have to be when he got the implants and had to hide them from sight under a doo rag. Look up the date of that interview and you'll be able to look at b4 and after picts for more clues

How things like that get out and Howard doesn't freak out kills me. He's either clueless or he's so proud of his ability to lie at will that he thinks no one would ever suspect that was a cover up.
