Neither do hair and wig people, cosmetologists, landscapers, stylists, chefs, personal trainers, and people who "help out" around the home - at least not in the aggregate. Still a drop in the bucket for billionaires. Normal people can't fathom that kind of wealth that literally cannot run out, you really can't spend that much. Howard could go to Sotheby's auctions wasted for a week and just keep buying stuff and still be rich.
