I think Dr Drew genuinely wants to help people. Its the premise of his show. While making a buck and putting together an entertaining show. Not to say he isn't exploiting them, but at least exploiting them to a possible better outcome.
Howard, on the other hand, owes his career to these misfits NOT improving their lives.
