I think hitting South Korea and Japan is the main concern. South Korea would be the easiest for them to hit, so they might take out Seoul before we reduce them to a crater.

If North Korea launched a nuclear weapon as an act of war it would be over for them. We would not nuke them back but they would be cut off from any country in the world. Even, China, who supports them now would be forced to cut off supplies.

It would be much worse for them than that. We have a huge military presense in South Korea. Attacking that country, especially with a nuke, would result in all out war. And yeah, at that point China would have nothing to gain by protecting them.
